Does it outline <?php ?> in all php files?

If a function has <?php ?> inside, it doesn't outline the function itself, just the tags.

Ah, right. Sadly this is limitation of Visual Studio, which cannot show region in contained language (PHP) containing a primary language (HTML). Meantime the only solution we can provide is to disable support for new HTML editor (Tools | Options | PHP Tools | Advanced).
We'll update our documentation with known limitations.
