What path are you looking for?

Visual Studio updates path to the icons file on every startup.
ProgId for that is "VisualStudio.php.11.0"

This is being updated when you start VS in registry in HKEY_CLASSES_ROOT\VisualStudio.php.11.0

After I fixed the path (just there), I did a test - closed VS, messed up the path and started it - it didn't fix it.

The key is right, but I am using Windows 8.1, I am NEVER REALLY administrator. I don't start VS as administrator by default. Let me test and get back to you.

Yep, running it as administrator fixed the paths. Strange, I shouldn't be forced to do this, there must be some elevated process during install...

Another thing. Running it as administrator did this:

Added this path:
C:\Users\vkosev\AppData\Local\Microsoft\VisualStudio\11.0\Extensions\15w5l0y0.den\CodeSnippets\Snippets\

3 more times, so the key (HKEY_CURRENT_USER\Software\Microsoft\VisualStudio\11.0\Languages\CodeExpansions\PHP) value is now:
C:\Users\vkosev\AppData\Local\Microsoft\VisualStudio\11.0\Extensions\15w5l0y0.den\CodeSnippets\Snippets\;
%MyDocs%\Code Snippets\Phalanger\My Code Snippets\;
