Now it's working. No idea what I did.

I got frustrated; started working on a different project (a C .vcxproj); accidentally started a second copy of vs2012; and noticed the PHP Tools start page was present. After that VS & PHP Tools behaved normally.

I've attached a log file from visual studio starting successfully with PHP Tools. Maybe you can tell something from what happens at record 31.

(this won't cause hanging, only Test Window for SQL shouldn't open. You can fix this by installing both x86 and x64 version of SQL tools)

Do you have VMWare installed? Some people reported VS hangs at start when vm_tray.exe is not running or "VMware Authorization Service" service is not running.
