"Add New Item" dialog is empty

I use PHP Tools for Visual Studio 2010 PERSONAL LICENSE 1.8.4608 (Preview Version).
I report a problem as below.
In solution explorer, I right-clicked folder and select [Add] - [New Item] menu then "Add New Item" dialog was shown.
But its content is empty such as a image attached.

You do not have the required permissions to view the files attached to this post.

Sorry but it was fixed after I tried to restart Windows and reinstall the plugin.

Thank you for reporting that. We'll try to replicate and we'll keep watching the issue.
Anyway I'm glad it is fixed for you for now. This may be caused by installing the plugin while Visual Studio is running.
Please give us know if it would happen again.
